This made me feel sick to the pit of my stomach....I've tried to sign it but 2 fields I don't understand what information they require as it's not in english, any idea what I need to put so I can sign it??

Hiya, from what i've been told ciudad/localidad is where you would put where you live ie. UK and paAs is for your postcode, that's how alot of people have been doing it and its worked for them (and me)
